Acyclovir , Zovirax®
Nucleoside Analog DNA Polymerase Inhibitor. M.W. 225.2.

  • Acute Treatment of Herpes Zoster (Shingles) Adults
         800 mg every 4 hours orally, 5 times daily for 7 to 10 days.
  • Acute Treatment of Varicella (Chickenpox) Adults  
         800 mg orally 4 times daily for 5 days.
  • Severe Disease Adults (complicated disease -e.g. disseminated infection, pneumonitis, hepatitis, meningitis or encephalitis) [1]
        
    5–10 mg/kg  IV every 8 hours for 2–7 days or until clinical improvement is observed, followed by oral antiviral therapy to complete at least 10 days of total therapy HSV encephalitis requires 21 days of intravenous therapy.

Adjust dose in patients with acute or chronic renal impairment

(200, 400, 800 mg tablets; 200 mg /5 mL suspension -Bottle of 1 pint -473 mL)

Breastfeeding : Even with the highest maternal dosages, the dosage of acyclovir in milk is only about 1% of a typical infant dosage and would not be expected to cause any adverse effects in breastfed infants. LACTMED 12/12/2015


 

Valacyclovir (Valtrex®)
Nucleoside Analog DNA Polymerase Inhibitor M.W. 324.3 .After oral administration, valacyclovir hydrochloride is rapidly absorbed from the gastrointestinal tract and nearly completely converted to acyclovir  

  • Acute Treatment of Herpes Zoster (Shingles) Adults:
         1 gram orally 3 times daily for 7 days. Therapy should be initiated at the earliest sign or symptom of herpes zoster .

Adjust dose in patients with acute or chronic renal impairment

(500 mg, 1 gram caplets)

Varicella zoster immune globulin (VariZIG) [2]

  • Indicated for for post-exposure prophylaxis of varicella in high risk individuals (pregnant women without evidence of immunity) within 10 days (ideally within 4 days) of chickenpox (varicella zoster virus) exposure . Adults.
         125 IU/10 kg of body weight intramuscularly into the deltoid muscle or the anterolateral aspect of the upper thigh, up to a maximum of 625 IU (five vials).
    • The intramuscular dose should be divided and administered in two sites, dependent on patient size.
    • Do not exceed 3 mL per injection site

(Single-use vial of 125 IU in 1.2 mL)
